In order to fully process your application, please include all of the information requested on the application form which includes two forms of ID per adult, 2 most recent employment paystubs per adult (copies are okay), and a $25.00 application fee per adult. * Note: This fee is required for all adults over 18 years of age. Your application will not be processed until the complete fees have been received.

All maintenance requests that are NOT emergencies need to be sent using the form below or called in at 801-695-2020 during business hours. PLEASE INCLUDE YOUR FULL NAME, ADDRESS, and PHONE NUMBER. Emergencies would include no heat in very cold weather or flooding due to a fault in the plumbing. For after hours emergencies such as these, please call 801-695-2020.
